POSITION SUMMARY
The Health Navigator is a non-certified position that will be trained to assist providers before, during, and after patient appointments. Once trained, this position will work independently under the direction of their supervisor and provider team to provide an exceptional experience for our patients.

PRODUCTIVITY AND EXPECTATIONS:
The Health Navigator position is an essential component to the success of HPCHC. This position is expected to prioritize time and responsibilities in the following manner:

85%: Care Coordination
• Navigator will assist patients with needed language or translation services.
• Work with the team to develop a patient-centered care plan.
• Use knowledge of client’s cultural, ethnic, religious, and social system to address issues and revise plan as indicated.
• Help patients with necessary paperwork to enroll in services and the facilitation of financial support.
• Assist patient as needed with transportation arrangements.
• Assess barriers to care for collaborative patients and implement and adhere to plans to reduce those barriers.
• Becoming a bridge for the gap between the patient and the medical system to accessing medical care.
• Provide a link to follow-up services.
• Reinforce patient self-management goals and coach to support lifestyle changes.
• Disseminate education and educational material to the patient, family, and community.
• Organize group education for the community and at HPCHC.
• Regularly review patient’s social determinants of health questionnaires and provide resource referrals as appropriate.
• Form partnerships with community-based organizations and assist patients with accessing community resources.
• Assist patients in accessing services and programs offered at High Plains.
• Outcome audit reports are reviewed and patients in need of care are contacted and encouraged to get their outcome goals current.
• Effectively communicates with patients and provider teams.
• Successfully communicates patient plans and concerns to providers and their teams.
• Manages referrals entered by provider per protocol, ensuring appointments are made, patients and consultants have the information for the appointment and results are received back to the clinician.
• Provides presentations to High Plains teams regarding patient outcomes. Works with grants specific to patient navigation services to meet benchmarks for patient care that are determined by the grant documents.
• Provide presentations to High Plains staff about deliverables and benchmarks of their patient panels.
• Communicate patient plans and concerns with healthcare providers.
• Collects and provides monthly data to funder, partners, and provider teams. Data is accurate, complete, and entered within 24 hours of patient interaction.

15%: General Duties
• Participate in Patient Centered Medical Home, Quality Assurance, Quality Improvement and Risk Management tasks per plans developed by HPCHC.
• Organize and provide training to staff members regarding programs offered at HPCHC.
• Complete required trainings to use motivational interviewing skills to assist patients.
• Complete required and provided patient navigation training.
• Complete online education modules as required.

QUALIFICATIONS
• High School Diploma or equivalent required. Associate’s Degree preferred.
• Complete patient navigation training and obtain certification within one year of employment.
• Current Basic Life Support Certification (BLS) or within 90 days of hire required.
• Prior medical field experience preferred.
• Prior customer service skills necessary to provide a positive experience necessary when they speak to patients and employees in person and on the phone is preferred.
• Experience working with a diverse population and/or underserved populations preferred.
• The position requires a working knowledge of modern office practices and procedures. This includes but is not limited to:
- Proficiency in using the Internet, Microsoft Office programs and general computer skills.
- Skill in operating office equipment such as copier, fax machine, scanners, etc.

GENERAL REQUIREMENTS

• Exercises professional judgment, courtesy, and professionalism in daily interactions with others.
• Works both independently and as part of a team; establishes and maintains effective working relationships with health care team members and diverse patient populations.
• Displays time management and organizational skills to consistently meet deadlines.
• Performs other duties as required.

There will be opportunities for other duties as this position may evolve and policies and training may change. This position utilizes proper body mechanics, transfer/lifting techniques, and appropriate equipment to minimize fall risk to patients and/or injury to self. This is an OSHA Category 1 position.
